Public Accommodation Request for City Program, Service, or Activity
The Office of Diversity and Inclusion handles ADA accommodation requests for City of Toledo residents and visitors. If you require special accommodations in order to fully participate in a city program, service, or other activity, please complete the online form below.
Request is Received
Diversity and Inclusion will review the request as described and ensure that all reasonable accommodations are provided. Though the ADA does not require the city to take any action that would fundamentally alter the nature of its programs or services, or impose an undue financial or administrative burden, the city strives to ensure that people with disabilities have an equal opportunity to enjoy all of its programs, services, and activities.
If you provided your name and contact information with the complaint, Diversity and Inclusion will follow up with you to let you know the outcome of the request.
If you are unhappy with a public accommodation request determination, you can file a grievance with the Office of Diversity and Inclusion.
