The City of Toledo will be hosting our Annual Health and Wellness Fair and Onsite Biometric Screening event to make it easier for you to care for your health and earn your incentives.

There will be lots of giveaways, raffle prizes, and a daily 50/50 CASH raffle!

How to Schedule Your Onsite Appointment

1. Log in to the portal or Wellworks For You Mobile App to access the appointment scheduler
2. Click on the 2025 Onsite Biometric Screening icon from the homepage
3. On the following pop-up screen, your preferred appointment date and appointment time from the drop down menus
4. You must confirm your appointment time by clicking CONFIRM SIGN UP in the top-right corner of the following pop-up screen.

Estimated Appointment Time: 12 - 15 Minutes

You will receive notice of your appointment time and an email confirmation 24 hours before your appointment.

Things to Remember

• Fasting Fingerstick panel to include Total Cholesterol, HDL, LDL, triglycerides, glucose, blood pressure, height, weight, waist circumference, and BMI
• You must fast (no food) for 9 hours before your appointment. Please drink plenty of water.
• Black coffee is permitted
• Continue to take any prescription medications
• If you have diabetes, please consult your physician before fasting

All Personal Health Information collected will remain confidential.

Your results will be uploaded to your Wellness Portal and 50 Points will be awarded within two weeks of your appointment.
